Parent/Guardian Consent, Assumption of Risk & Liability Release
Trinity Lane at The Zen Loft inside Uplifting Connections
I, the undersigned parent and/or legal guardian, understand and acknowledge that my child is participating in a voluntary children's class, activity, program, workshop, or event offered by Trinity Lane at The Zen Loft inside Uplifting Connections.
1. Parent/Guardian Must Remain With Child
A parent or legal guardian must remain with their child at all times throughout the entire class or activity. This is a parent-and-child enrichment program and is not a drop-off child-care program. A parent or legal guardian must remain on the premises and actively available to their child for the entire duration of every class. The parent/guardian remains responsible for the child's supervision, toileting, feeding, behavioral needs, comfort, and other basic care. Children may not be dropped off or left unattended.
Children may not be dropped off or left unattended at The Zen Loft, Uplifting Connections, or any location where a Trinity Lane children's class or activity is being held.
The parent or legal guardian must remain on the premises and immediately available to supervise and assist their child for the full duration of the class or activity, including arrival, participation, breaks, and departure.
Trinity Lane, Christina Lane, The Zen Loft, Uplifting Connections, Maureen Hancock, instructors, staff, volunteers, and representatives do not provide babysitting, drop-off care, custodial care, or licensed child-care services as part of these children's classes.
If a parent or legal guardian leaves the premises at any time during the class, the child may not remain at the class without another authorized parent or legal guardian present. Trinity Lane reserves the right to require the child to leave the activity if the required parent/guardian supervision is not maintained.
2. Nature of the Setting
I understand and acknowledge that The Zen Loft inside Uplifting Connections is not a fully childproofed or child-care facility. The environment is a wellness and event space and may contain furniture, fixtures, flooring, sound-healing instruments, meditation props, decorations, electrical equipment, doors, stairs, uneven surfaces, and other items or conditions that may not be specifically designed or secured for young children.
I understand that children's activities may include music, movement, meditation, mindfulness, sound, gentle physical activities, crafts, and/or the use of wellness-related instruments and materials. Some activities may involve ordinary risks associated with movement, interaction with the physical environment, or participation in a group setting.
3. Parent/Guardian Responsibility and Supervision
I acknowledge that I am responsible for my child's supervision and safety at all times while attending a Trinity Lane children's class or activity.
I agree to actively supervise my child and remain attentive and available to respond to my child's physical, emotional, behavioral, sensory, or other needs throughout the class.
I understand that the presence of a Trinity Lane instructor or facilitator does not transfer my parental responsibility or replace my required supervision of my child.
4. Assumption of Risk
I knowingly and voluntarily assume all risks associated with my child's participation in the class or activity, including risks that may be foreseeable or unforeseeable.
Such risks may include, but are not limited to, slips, trips, falls, bumps, collisions, minor injuries, interaction with furniture or equipment, use or proximity to sound-healing instruments, movement-related injuries, allergic or sensory reactions, and other risks inherent in participating in a children's wellness, educational, recreational, or enrichment activity.
I understand that children may behave unpredictably and that accidents may occur despite reasonable precautions and parental supervision.
5. Release and Waiver of Claims
To the fullest extent permitted by Massachusetts law, I, on behalf of myself and my child, hereby release, waive, discharge, and agree to hold harmless:
• Christina Lane, individually;
• Trinity Lane;
• The Zen Loft;
• Uplifting Connections;
• Maureen Hancock, individually; and
• each of their respective owners, operators, instructors, employees, agents, representatives, volunteers, contractors, and affiliates,
from claims, demands, actions, causes of action, damages, losses, expenses, or liabilities arising out of or related to my child's voluntary participation in the class or activity, including claims arising from the ordinary negligence of a released party, to the fullest extent such a release is permitted by Massachusetts law.
This release applies to claims for personal injury, illness, property damage, or other loss arising from or related to my child's participation in the activity or presence at the premises.
6. Limitations of Release
I understand that this agreement is intended to provide the maximum protection permitted by Massachusetts law and is not intended to release or waive liability that cannot legally be waived.
Nothing in this agreement is intended to release, waive, or limit liability for conduct that Massachusetts law does not permit a party to release or waive, including, where applicable, gross negligence, reckless conduct, intentional misconduct, or other liability that cannot legally be excluded.
7. Medical Treatment
I understand that Trinity Lane and the individuals identified above are not medical professionals and that the children's class is not medical treatment, therapy, or medical care.
In the event of an emergency involving my child, I authorize reasonable emergency assistance to be obtained, including contacting emergency medical services when deemed necessary. I understand that I am responsible for any resulting medical expenses.
8. Acknowledgment of Voluntary Participation
I understand that my child's participation is voluntary. I have had the opportunity to ask questions regarding the activity, the environment, supervision, and potential risks.
By signing below, I acknowledge that I have read and understood this Parent/Guardian Consent, Assumption of Risk & Liability Release, that I understand its contents, and that I voluntarily agree to its terms as a condition of my child's participation.
